Notes:

1. All Varsity and Junior equipment must be picked up at Mother Teresa Catholic High School on 1065 Sunningdale Road East, London. In the small gymnasium.
   - The pickup dates will be Saturday, April 16 for Varsity, and Sunday, April 17 for Junior.
2. Bantam equipment must be picked up at the London Minor equipment area at 100 Bessemer Court, Unit # 1, London.
   - The pickup dates will be Monday, April 18, and Tuesday, April 19 for Bantam
3. Please remember to bring your player fee $475.
   We accept Cash, Debit, Credit and Cheques (cheques cannot be post dated) for player fees. PLAYER FEE IS NON-REFUNDABLE. The player fee includes; busing to/from all road games (for players on the road trip roster) and equipment usage fee. In addition, players will receive; a jersey, game socks, and a mouth guard. Due to the time frame and nature of the league fees, there will be no refunds. Please make sure you want to play summer football prior to accepting membership.
4. Once a fee has been collected, the fee will NOT be refunded.
5. Players are required to sign an Equipment Loan Contract Equipment (provided at the equipment pickup) must be secured with a Credit Card, card holder must be present to sign the contract along with the player. The equipment is valued at $500.
   This deposit will not be cashed unless equipment is not returned.
6. The equipment return date, ONE DATE ONLY, is Monday August 22nd. A specific time will be provided on the London Falcons web site. When equipment is returned, at the return date, in good clean condition then the $500 deposit credit card slip will be returned. If equipment is not returned or lost or damaged your $500 security deposit will be processed. If equipment is returned after the return date and the security deposit has been deposited then a 10% ($50) surcharge will apply. If equipment (game/practice pants) is returned not cleaned a cleaning charge of $25 will be accessed.
