Notes:

1. All Varsity and Junior Varsity equipment must be picked up at Mother Teresa Catholic High School on 1065 Sunningdale Road East, London

2. All Pee Wee equipment must be picked up at the London Minor equipment area at 100 Bessemer Court, Unit # 1, London
   Links: PDF file for information and map and Google Map to LMFA Area
3. The pickup dates will be May 3 for Varsity, May 4 for Junior, and April 26 for Pee Wee - at the times shown below.
4. Please remember to bring your player membership fee ($350). Player membership fee is non-refundable. For information please see team policies.
5. An Equipment Loan Agreementwill be provided. The Equipment Loan Contract must be signed by both player and parent prior to receiving equipment. The equipment is valued at $500.
   - MasterCard, VISA or American Express must be used as the security deposit ($500) for the equipment.
   - This deposit will not be cashed unless equipment is not returned.
6. The equipment return date (one date only) is Monday August 18th. A specific time will be provided.
